How to explain a Case Manager to your child

A healthcare case manager spends their days talking with patients, doctors, nurses, and families to make sure everyone gets the help they need. They might use a computer to check when someone has a doctor's appointment, call a pharmacy to set up medicine delivery, or help a family find a ride to the hospital. They keep lists and calendars, make phone calls, and ask lots of questions to understand what each person needs. For example, if someone is leaving the hospital after being sick, the case manager makes sure they have a safe place to go home to and all the right medicines waiting for them.

This work matters because when people are sick or hurt, everything can feel confusing and scary. The case manager is like a helpful guide who makes sure nothing important gets forgotten—like making sure a child with asthma has an inhaler at home, or that grandma has someone to help her take a bath safely. They connect people to the right helpers, so patients can focus on feeling better instead of worrying about all the details.
